To book on-line:
1. If you haven't already, choose a clinic you'd like to book from our Clinic Schedule, then come back to this page.
2. Read our Terms and Conditions.
3. Click here to fill out and send in your registration form.
4. Click below to submit your deposit.
By clicking on the Buy Now button, you are confirming that you have read and understood the Terms and Conditions.
5. You will receive immediate confirmation of your booking and deposit via email.
To book offline:
1. If you haven't already, choose a clinic you'd like to book from our Clinic Schedule, then come back to this page.
2. Read our Terms and Conditions.
3. Click here to fill out your registration form. You will have the option of emailing and/or printing your registration form.
4. Print off the form and send it in with your deposit. If you email your registration form, you still MUST send in a copy of it along with your deposit.
By sending in you deposit, you are confirming that you have read and understood the Terms and Conditions.
5. We will reserve your spot in the clinic when we receive your registration form (paper or email). Your reservation is valid for two weeks or until the deposit is received, whichever occurs first.
